This "product" must be added your order in order to receive the extended warranty.

An extended warranty registration form will be emailed to you following your purchase. Your completed form must be emailed back to us within 6 weeks of your purchase. If the completed form is not emailed to us within 6 weeks, the extended warranty will be voided.
